CHADIS FOR DEVELOPMENTAL SCREENING
- Please register at CHADIS to complete developmental questionnaires online prior to your child’s well check.
- If your child is between 9 and 36 months and you have never done this, please complete baseline “well visit” forms at CHADIS.
- If you have done this already, the ages we repeat screening are 9 months, 18 months, 30 months.
To register online through Chadis:
2. Create a user via “New User” for your child using this Invitation Code: 8054803730. Fill out the registration information.
3. Choose visit type as ‘‘Well Visit” unless told otherwise. Fill out the assigned questionnaires.
a. If time is limited, the most important questionnaires are “Ages and Stages” and the “MCHAT” (17 months to 3 years).
b. You only need complete family history once.
